*Registration for 5150 training and exam is paused until January 2025.*
Once registration resumes, follow these steps to register for 5150 certification training and exam:
- Complete and submit a 5150 training authorization request form via DocuSign at least 10 days before your desired training date.
- DocuSign will route the request form to your supervisor and director for their authorizing signatures.
- After they submit their signatures, we at Learning Partnership will process your request within 3 business days.
- Your training registration is successful when you receive an email from Learning Partnership staff. The email will confirm the training date you are registered for and will instruct you to sign up for an exam session. You will also receive an email from DocuSign containing a link to your completed request form that confirms your training date.
- Once you have received your training registration confirmation email, you are eligible to sign up for an exam session. Simply respond to the training registration confirmation email that you receive to sign up for your exam session.
