Transformational care planning (TCP) is a person-centered and family-driven service planning model which builds on the principles of inclusion, hope, wellness, resiliency, and recovery. It is a collaborative process between an individual/family and his/her service provider(s). This important best practice is largely based on principles derived from the book Treatment Planning for Person-Centered Care by Neal Adams and Diane Grieder.
