Exam results

How will I know I passed the exam?
If you passed the exam, you should receive your card via email from DocuSign about two weeks after your exam date. 

If you did not pass the exam, you should receive an email with the result within two weeks of your exam date. 

What can I do if I don't pass the exam?
You can retake the test two more times and pass by scoring at least 80 percent. If you still do not pass, you can register for another training and exam.

Writing holds when your status changes

Can I write holds if I know I passed the exam?
Even if you were notified that you passed the exam, you must hold a valid 5150 card before you can write holds. The law allows only those with permission of the Behavioral Health Director of the County to write holds. Consequently, the signature on the card you receive is your permission. You cannot write holds until you have received your card unless you have another valid card in your possession.

What if I move from one agency to another?
The 5150 authorization card is only valid when used to perform duties for the agency that is listed on your 5150 card. This means if you currently hold a valid 5150 card, and are joining the Behavioral Health Services team, you cannot use a 5150 card from another agency. You will need to email a new application from your new employer to [email protected]. You will not need to retake the training or the test. Once your application has been approved, we will email you a new 5150 card within two weeks. 

If you change organizations, you can transfer your 5150 card writing privileges without retaking the class. You will need to email [email protected] that you have changed agencies and submit a new application. The County must recognize both agencies as a 5150-writing authority. 

What if I work at two different agencies where 5150 holds are allowed to be written?
You will need to complete an application form for each agency and send them in together when you sign up for the class.

Exceptions 

I am an associate level, unlicensed staff, psychiatric technician, or licensed vocational nurse and need a 5150 certification. Do I need to take additional steps to register for 5150 training and exam?

You will need to submit a request from your immediate supervisor that describes:

  • Why this exception is required by your organization and why a licensed staff cannot write these holds. 
  • The experience and education that makes you qualified to write a 5150 hold. 
  • Who you can call 24-hours a day for supervision. This person must be licensed with a valid 5150 card. Include their name, phone number, 5150 card expiration date and license​ number and type.
