You have up to two (2) clients without an MRN. | Call us at 1-800-704-0900 Share with the Call Center rep as much information as possible including address and phone number. At minimum, you need to share this information in order for a screening episode to be created: - First name
- Last name
- Date of birth
- Sex
- Social security number, if applicable
- Preferred language
- Ethnicity
| We will look up the client in myAvatar. If the client is in our system, we will provide the MRN. If we can't locate the client in myAvatar, we will create a Screening Episode, which will generate a MRN for the client. |
You have three (3) or more clients without an MRN. | - Download and complete the Client missing MRN template 08/24/22.xls.
