Request to merge charts 

When trying to map a client, you may notice that the client has multiple medical record numbers (MRNs).

To identify the appropriate MRN for mapping, email us all the MRNs you see for this client with any additional information and we will help you resolve the issue.

Request to delete charts 

If a client record was created by a CCP, the client record needs to be deleted. Email us the information below and we will help you resolve the issue. 

• MRN
• Client's first and last name
• Date of birth
• Reason for deletion
