Scenario 1

  1. Admitted to the 921 S. 1st – Rise on 07/12/2021
  2. Admitted to Wool Creek – F&C OPC on 07/28/2021.
  3. Discharged from 921 S. 1st – Rise on 08/01/2021 (after Wool Creek – F&C OPC admission date)
  4. Discharged from Wool Creek – F&C OPC on 08/30/2021.

If the primary diagnosis for the first service program is the same as the second service program, the CCP can either: 

  1. Leave the diagnosis as is.
  2. Add the same primary diagnosis with the second admission date.

 

 

 

If the diagnosis for the second service program is different from the first primary diagnosis, add the second primary diagnosis.
