Annual Audit of Providers
Purpose of the Annual Audit of Providers is to ensure that BHSD Provider Network is complying with Federal and State regulations in providing services to Medi-Cal members such as documentation, processes to identify fraud, waste, and abuse, and ensure quality and efficient care.
- To provide guidance and ongoing monitoring of the BHSD Provider Network’s compliance with Mental Health Plan and Drug Medi-Cal Organized Delivery System requirements.
- To identify strengths and areas of improvement and support the BHSD Provider Network to provide quality care to Medi-Cal members.
- Each County Contracted Provider and County Clinic will have a primary and secondary provider liaison to streamline communication, expectations, and reduce multi-staff outreaches
- Provider liaisons will facilitate the administration of the annual audits by working with providers throughout the process and coordinate with internal BHSD subject matter experts related to programs and regulations.
- Providers will be given an audit schedule that identifies the key deliverables and timelines for the audit process.
- Audit tools will be shared with providers including instructions on how to complete the tools and securely upload the supporting documentation. These tools will include administrative requirements such as policies and procedures, personnel files, chart files, and facility review as applicable. Please refer to the tools for details.
- The sampling of personnel files and chart files will be using the 8/30 methodology. This methodology aligns with the documentation and clinical standards under CalAIM. Please refer to the provider instructions for additional information.
- Once documents have been reviewed, the results of the review will be communicated to providers including any items that will require corrective action.

BHSD LPS Designation
QM – LPS Designation is facilitated by the Provider Relations Operations (PRO) Team. PRO conducts SMHS site certification and LPS Designation reviews to determine if the entity has demonstrated an infrastructure to support provision of behavioral health services through review of required agency policies, procedures, licenses and other supporting documentation. Prior to the provision of involuntary psychiatric treatment and evaluation services, all facilities that have or are seeking 5150/5585.50/5751.7 Lanterman-Petris-Short (LPS) Designation that are located in Santa Clara County must:
- Maintain all applicable current licenses, certifications and accreditations appropriate for the facility type.
- Request and obtain LPS Designation by the Santa Clara County Behavioral Health Services (BHS) Director or designee.
- Receive approval from Santa Clara County Board of Supervisors.
- Have state Department of Health Care Services authorization Confirmed.
- Submit quarterly Phase III 929 reports.
- Beginning 1/1/26, begin reporting SB43 new definitions of grave disability – due to severe substance use disorder or grave disability due to both a mental health and a substance use disorder.
- Implement AB2242 model care coordination plan for Lanterman-Petris-Short (LPS) Act-designated and approved facilities minimum required elements to ensure continuity of services and care for all individuals exiting involuntary holds.
- Facilities will be reviewed for re-designation every three (3) years at a minimum.

- Crestwood Manor Psychiatric Health Facility

- Kaiser Behavioral Health
The BHSD Director appoints a multi-disciplinary LPS Facility Designation Team, which consists of representatives both employed by Behavioral Health Services and those with interests in the services provided in Santa Clara County:
- Provider Relations Operations
- BHSD Medical Director or designee
- Inpatient Quality Improvement Division
- Public Guardian
- County Counsel
- The Law Foundation of Silicon Valley
- Licensed Clinical Professional, can be CCP partner
- Age and/or Service Specific Contract Monitor or Division Lead
- NAMI
- Consumer(s)
Based on extensive multi-disciplinary team review, the LPS Facility Designation Team makes recommendations to the BHSD Director. The Behavioral Health Director may then forward the recommendations to the Santa Clara County Board of Supervisors for approval prior to DHCS submission.
Entities seeking LPS Facility Designation are provided a Protocol which identifies the categories and materials the Team will review prior to a site visit. The PRO Team reviews all of the submissions, other team members focus on Patient Rights policies, procedures and processes. Please see sample protocol below.

LPS Designated Facilities are responsible to submit quarterly reports related to individuals served in their facility. Reports are due by the 15th of the month following the quarter’s end, January, April, July, October. Please see sample below.

BHSD Site Certification Training
Who needs certification?
- Any participating provider who provides SMHS regardless if this is only a small part of their award (MHSA and SMHS)
- Any participating provider that serves at a location where SMHS are routinely offered – be it 5 days a week or a half day a week, 24 FTE staff or .25 FTE staff.
- Schools project – 80 schools where services were provided without certification.
- Community based services - Any participating provider making claims in Avatar and some residentials where BHS files claims on their behalf
- Certain residential facilities, 16 beds or less, or larger facilities with enhanced adult residential
