Care coordination and multidisciplinary teams

Case managers must not only have many of the same abilities as other professionals who work with substance abusers (such as counselors), they must also possess special abilities relating to such areas as inter-agency functioning, negotiating, and advocacy. We will spend time taking a close look at the roles and functions of case management, and the challenges, and the case management role specifically in Pre-treatment primary treatment, and aftercare. We will incorporate discussion, lecture, video, and interactions so we will be requesting cameras on!

Learning objectives:

  • Identify nine referral and service coordination activities which include engagement; assessment; planning, goal setting, and implementation; linking, monitoring, and advocacy; and disengagement.
  • Identify the five elements associated with becoming a culturally competent case manager.
  • Describe elements of case management service delivery during the Pre treatment, Primary treatment, and Aftercare phases of service delivery.
  • Develop increased documentation language for Case management documentation.
