BH-CONNECT

The Behavioral Health Community-Based Organized Networks of Equitable Care and Treatment (BH-CONNECT) initiative is designed to increase access to and strengthen the continuum of community-based behavioral health services for Medi-Cal members living with significant behavioral health needs.

BH-CONNECT is comprised of a new five-year Medicaid Section 1115 demonstration, State Plan Amendments (SPAs) to expand coverage of evidence-based practices (EBPs) available under Medi-Cal, and complementary guidance and policies to strengthen behavioral health services statewide. There are 4 overarching goals of BH-Connect:

  1. Enhance the continuum of care: Offer a broader range of community-based services to meet diverse individual needs.
  2. Standardize and scale evidence-based practices (EBPs): Use proven treatments to improve recovery outcomes.
  3. Address disparities: Direct resources to populations/communities that historically faced barriers to accessing care.
  4. Meet specific mental health needs: Address needs of children and youth involved in child welfare, and individuals who are justice-involved and/or experience homelessness.

The County of Santa Clara Behavioral Health Services Department is working with internal subject matter experts and external partners to implement these initiatives as it pertains to community members requiring behavioral health services throughout the county.  This webpage serves as centralized resource center for county staff and contracted community providers. 

Townhalls

BH-CONNECT Bimonthly Town Halls will be held every second Friday at 11:00 AM to share updates and discuss all things related to the BH-CONNECT initiative.
